In normal circumstances, I try to keep caught up so that I can reply within a week. My current reply pile goes back months, and I'm only now trying to tackle it because of holidays and work and life. My current goal is to get caught back up in time for InCoWriMo, so I'm not completely buried in correspondence.

I try to reply within 1-2 months (I aim for 2-4 weeks) but sometimes I move the reply pile around on accident and find correspondence well after that timeframe. Holidays and InCoWriMo change the reply frequency/timelines significantly. I'll drop a postcard if I'm traveling or really behind to notify someone that a letter is on the way, or if I find a nice postcard for someone who has an interest.
Cards usually don't get replied to unless there is a letter is attached or there's something I can respond to - exceptions are made if it's an unexpected card from outside the USA with a little writing as some countries have crazy international postage rates that make larger correspondence expensive. For instance, someone sent me a postcard from Zurich airport and it cost them 2 CHF (about US$2.10/£1.50) on postage alone! That'll get replied to if there's an address to do so.
